Four-Day Intensive Care for Complex Chronic Pain

A comprehensive, in-person treatment program in Clermont, Florida for individuals who have struggled with ongoing pain and have not found lasting relief.

This structured four-day program was developed by Dr. Joseph Jacobs, a Doctor of Physical Therapy and inventor of ASTR.

What Happens During the Four Days

Day One
Treatment begins immediately. Dr. Jacobs will spend most of that session releasing scar tissue, fascia restriction and inflammation with the various tools he has developed. He will also review nutrition and lifestyle factors that may be contributing to pain.

Days Two–Four
Each day includes continued hands-on treatment, along with guidance on posture, daily habits, movement patterns, and specific exercises to support lasting improvement.

Each session lasts approximately 1 hour and 40 minutes.

This is a comprehensive and holistic approach to patient care. It is structured, focused, and designed for individuals who are ready to address the deeper contributors to chronic pain.

Why a Four-Day Format?

Our average client is dealing with complex, chronic pain. They typically have significant scar tissue, fascia restriction  and inflammation when they begin our program; as well as lifestyle and nutritional habits that are significantly affecting their quality of life.

Our goal for every single client is that they walk out of our clinic by day four with at least an 80% improvement in their pain levels, range of motion, mobility and strength. 

In order to achieve this goal, we have found that focused, consistent, hands on care over the course of four days consistently produces the outcomes we desire as a team. As a result, our clients are satisfied with the time and investment they make in traveling to see us.

$4,000 Four Day Intensive

This program is private-pay and is not billed to insurance.

The first step is the $500 Telehealth Evaluation with Dr. Jacobs. During this one-hour consultation, he reviews your history, prior treatments, and overall presentation to determine whether your case aligns with the Four-Day Intensive model.

If the program is not appropriate for your condition, we will tell you directly. Our priority is clinical alignment, not enrollment.

The Four-Day Intensive is structured specifically for complex and unresolved cases. Rather than brief, isolated visits spaced weeks apart, this format allows for concentrated assessment, corrective progression, and neurological integration across consecutive days.

Many patients who apply have already completed standard physical therapy, injections, or surgical procedures without lasting resolution. This program focuses on identifying layered mechanical and neuromuscular contributors that may not be addressed in conventional care models.

The structure, duration, and sequencing of treatment are intentional and designed for complex presentations.

For most patients, one Intensive produces meaningful improvement and a clear corrective direction.

For others with complex or long-standing conditions, additional care may be recommended. This is determined individually and discussed transparently during your evaluation and treatment days.

At the conclusion of the Intensive, you will receive a structured summary of findings, progress made, and recommended next steps.

Some patients transition to self-management. Others benefit from periodic reassessment or coordination with a local provider. The goal is long-term stability rather than ongoing dependency.

Not every case is appropriate for this format. If your case is not accepted, we will provide guidance on alternative options when possible.

The Four-Day Intensive is a private-pay program and is not billed to insurance. Its extended session length and concentrated structure fall outside traditional reimbursement models.

Documentation can be provided upon request should you wish to pursue possible out-of-network reimbursement independently.

Yes. MRI reports, X-rays, surgical notes, and other records may provide helpful context.

However, candidacy is determined primarily by your current presentation and functional assessment rather than imaging findings alone.

Many patients travel to Clermont, Florida for this program. Upon acceptance, our team can provide general recommendations for nearby accommodations.

Patients are responsible for arranging their own lodging and transportation.
